(a) A pre-filled advance directives form: You can print and sign and give one copy to your doctor and keep a copy for yourself and your loved ones.
(b) A letter to your doctor that expresses your values, wishes and preferences about care you wish to receive in the future when you are in the last chapter of your life.

ABOUT US:


Our research has shown that while almost all doctors agree that it is important for them to have end-of-life conversations with their patients, most doctors struggle with these conversations. Our research has also shown that patients from all ethnic backgrounds agree that it is important for them to have end of life conversations with their doctors and to complete advance directives. However, patients do not quite know how to initiate these conversations or complete advance directives.
Our goal is to empower all adults to take the initiative to talk to their doctor about what matters most to them at lifes end. We also want to help people complete their advance directives. Advance directives are legal documents that allow you to describe your decisions about end-of-life care ahead of time. They help you convey your wishes to family, friends, and health care professionals and to avoid confusion and conflict later on.
This App will ask you a few simple questions to elicit your values and preferences for care at the end of life. Your responses will be used to create a pre-filled advance directives form and a letter to your doctor. The project is based on extensive research done at Stanford and is specifically designed to help you voice the key information your doctor needs in order to provide end-of-life care to you that is congruent with your values and wishes.
Use our App, answer a few questions and print out your Advance Directives and discuss it with your family and your doctor. It is free, easy, and extremely important.

NOTE: Please remember to print and save your documents after you complete the form. If you do not save and print the document after completing it, the information will be lost and you will have to re-answer the questions. We do NOT save any of your information on the App.
